Client Experience Survey

Provide your feedback in a 20 minute online survey and receive a $20 gift card

Feedback helps us to review our performance and continue to improve our services.

To do this we have developed the Client Experience Survey – to get feedback directly from the young people we work with. People like you.

The survey is a 20 minute online call with a YSAS staff member who will ask you questions about your experience with our services.

You will be reimbursed with a gift e-card on completion of the survey.

To complete the survey, you need to book a 20 minute online interview with a staff member using the form below.

The staff member won’t be someone from your site, service or program. Interviews will be confidential and will not contain any identifying details.

Your answers will help us to identify: 

  • The strengths of the service,
  • Areas for improvement,
  • Impact of past improvements
  • and An action plan for further service improvement.

The results will be distributed to all staff in the respective service area. Youth and consumer advisory panels will also get a copy for further commentary and to ensure our accountability to service users and the broader community.
